Basic Info
What ages do you accept? Do you accept children?
We accept patients 5 years of age and older. We do not currently provide routine vaccinations for our patients; however, we will work with you to arrange necessary vaccines.
What is the Direct Primary Care (DPC) model?
Direct Primary Care (DPC) is an innovative alternative payment model for primary care being embraced by patients, practitioners, employers, payers and policymakers across the United States. The defining element of DPC is an enduring and trusting relationship between a patient and their primary care provider.
Is DPC the same as Concierge Medicine?
Concierge medicine is traditionally defined as a relationship between a patient and primary care physician in which the patient pays an annual fee or retainer. This fee can range anywhere from $2,000 to $30,000 a year. Many concierge doctors also bill insurance or Medicare for actual medical visits.
DPC is an insurance-free model which offers significant savings in addition to concierge-level care.
What do I get with my DPC membership?
You’ll receive unlimited office or telemedicine visits, medication refills, lab referrals, discounted injections and infusion therapy treatments, an annual wellness exam, and 24/7 access to a provider.
Do I have to be a DPC member to be seen by one of your providers?
No, not at all. Direct Primary Care membership is just one of the more popular services that we offer. However, all of our services are available to our patients whether they participate in the DPC membership or not.
Is Direct Primary Care considered health insurance?
No, DPC is not insurance. The monthly service fee is your payment for ongoing primary care services offered by your provider. We strongly recommend that patients do have insurance for care that may be needed beyond what we offer (specialty care, ER or hospitalization, surgery, etc.). Direct primary care couples nicely with a high deductible “wrap around” or “catastrophic” policy.
What is your payment policy?
Since we are a direct patient care clinic that does not accept insurance, we require payment at the time of service. We accept cash and credit/debit cards. Each patient will be provided with a superbill, upon request. You may present this form to your insurance for reimbursement, subject to your insurance’s policy. Those with high-deductible insurance and/or no insurance find this type of service especially helpful.
Do you accept any form of insurance?
No, we do not accept or bill health insurance. In the DPC model, we contract directly with the patient, not an insurance company, to provide quality care. By eliminating the restrictions and frustrations associated with health insurance, we keep costs low and spend as much time as needed with our patients.
I have Medicare. Can I still be a patient of Evolution Health & Wellness?
Absolutely! You will need to sign a one-time waiver declaring that neither you nor our office will directly bill Medicare for our services. You can continue to use Medicare for laboratory testing, imaging, and medications prescribed by our office, as well as specialty care and hospitalizations. You may even find that you will save money on your primary care, labs and medications through our discounted pricing.
How long is my membership contract?
Evolution Health & Wellness operates on a monthly billing basis. There are no long-term contracts, so you may terminate your contract at any time. But, before you do, please reach out to us. *Fees may apply to re-register.
When will billing begin for my membership?
Payment for the first month will be taken at registration. Subsequent payments will be due on the 1st day of each month.

What types of services does your clinic provide?
Evolution Health & Wellness offers primary care, urgent care, injection therapy, IV therapy, pre-employment exams, school and sports physicals, smoking cessation treatment, weight management counseling and more.
What types of exams and physicals do you provide?
We offer preventive screenings, routine physical exams, school, sports, and camp physicals, foster care physicals, pediatric health exams, excluding immunizations, pre-employment exams, and men’s health exams.
What is a telemedicine visit? How will it benefit me?
A telemedicine visit is a medical visit that takes place online. We use state-of-the-art technology for each visit so that our providers are able to see clearly and interact with patients more efficiently. During a visit, you’ll be asked questions about your symptoms and medical history, just like the ones you’d hear in a doctor’s office or an urgent care center. The biggest difference is that your visit takes place online and from the comfort of your home.
What kind of diagnostic tests do you take?
We perform Rapid Strep Tests, Influenza Swabs, Random Glucose Checks,
Urine Dipsticks, Urine Culture & Sensitivity Tests, Wound Cultures, Electrocardiograms (ECG), Pregnancy Tests, and More.
What's included in an urgent care visit?
Your visit will include a customized treatment plan from a certified nurse practitioner. The treatment plan includes a diagnosis, recommended medications and prescriptions, if necessary, referrals, and follow-up related to the visit.
Do you prescribe birth control?
Yes, our providers will help you choose the right type of birth control for your situation. Patients 13-50 years old can schedule a visit for birth control refills. If this is your first time taking birth control, a 3-month supply will be sent to your local pharmacy. Those previously using birth control will receive a one-year supply, with a blood pressure reading. We will advise you on ways to get that reading to us.
What types of medication do you prescribe?
Our nurse practitioners can prescribe medications for most common medical conditions. Your treatment plan may include over-the-counter or prescription medications like antibiotics, antivirals or antifungals. We also prescribe birth control and emergency contraception.
Will my provider discuss alternative treatment options with me?
Yes! Our providers use a variety of approaches to treat their patients. Alternative treatment methods are discussed during every visit so that patients can make the best decisions regarding their health care.
Can I contact my provider after hours?
Yes, you may call, text, or email your practitioner after hours. Your practitioner will respond to you as soon as possible. If you should experience a medical emergency, call 911 for immediate assistance.
What if I need medical attention while I'm away from home?
We are here to help to provide you care where you are. Because we’re familiar with your medical history, we can streamline your care when you’re sick away from home. In the age of telemedicine, many illnesses can be diagnosed and treated with a simple conversation by phone or webcam. We’ll locate the nearest pharmacy and order the medications most appropriate for your present circumstances.
Will my visit be kept confidential?
Your health and privacy are our top priority. Your online visit and results are HIPAA protected at all times. Remember, you control your results. You choose to release your results to us.
